In collaboration with the Saw Swee Hock School of Public Health, we designed Careverse, a mobile application created to make difficult end-of-life conversations more approachable and constructive.
The app guides patients, caregivers, and healthcare professionals through a thoughtful series of questions, capturing the values, wishes, and priorities that matter most to the patient. These insights are compiled into a clear, concise report for doctors, ensuring that care decisions are aligned with the patient’s and family’s true needs.
By bridging communication gaps in these sensitive moments, Careverse empowers all parties to navigate end-of-life planning with empathy, clarity, and respect.
